  Care Services
   
 

We offer comprehensive support services to people living with HIV disease in Sonoma County. We walk you through the maze of health care services and help you address your health and social service needs. We provide benefits counseling, case management services, access to in-home care, low income housing, support groups and volunteer services. Our team of case managers are led by social workers and nurses with extensive experience in HIV care. All our services are provided free of charge and are completely confidential. 

Case Management
You will be matched with a case manager to help you access the resources you need. We can help you gain access to information and referral, home health aides, housing search assistance, emergency financial aid, HIV-specific medical care, counseling and support groups.

Benefits Counseling
Our skilled staff will guide you through the application process for various programs and assist you in resolving problems you may encounter while applying for benefits. 

We can help you determine your eligibility for benefit programs such as Social Security, MediCal, MediCare, disability, health, and life insurance, and more. We assist you with the application process and filing claims.

Housing Search
We assist eligible clients in obtaining permanent housing and emergency rental assistance.

Legal Referrals
We can provide referrals to attorneys to help with legal issues, wills, and durable power of attorney forms.

Attendant Care
When someone needs in-home care, a home health aide can help with meal preparation, grocery shopping, laundry, light housekeeping, and errands.

Volunteer Services
You can request help with getting transportation to and from medical, dental and therapy appointments, in-home practical support, moving, and personal coaching.

Information & Referral
We offer information, referrals, and HIV-specific resources such as food banks, Pets are Loving Support (PALS), medical care, support groups, children's services, and counseling. This is available to anyone in the community.
