At Face to Face, we show up for our community with one part fierce compassion and one part practical care. Every day we open our doors – and our mobile van – to Sonoma County residents, especially those navigating multiple health and social challenges that affect their well-being.

The Challenge
At Face to Face, we recognize that health challenges rarely exist in isolation. HIV, STIs, substance use, mental health struggles, housing instability, and financial hardship are deeply interconnected – each one making the others more likely and more difficult to address. Traditional healthcare systems often treat these issues separately, creating barriers for people who need multiple types of support. That’s why we lead with love and practical care, offering integrated services that meet people where they are. At Face to Face, individuals who come to us for sterile supplies have access to a pathway to HIV/HCV/Syphilis testing. Those who qualify, are HIV/AIDS diagnosed and may access housing support and benefits of navigation. Our mental health program is open to our participants. All services ensure compassionate care – without judgment. By combining fierce compassion with practical support, we help people access the care they need while maintaining their dignity and autonomy. This comprehensive, person-centered approach doesn’t just improve individual health outcomes – it strengthens our entire community’s wellbeing.
Our community faces interconnected health crises that demand an integrated response:
- Rising drug use and overdose rates exceed those of many urban areas
- Limited access to HIV and STI prevention and treatment
- Growing housing and financial instability and displacement
- Mental health needs outpacing available services/complex systems that create barriers to accessing care

Our Approach
We meet you where you are – literally and figuratively. Our diverse staff meets the unique needs of marginalized populations, including those living with HIV, LGBTQ+ individuals, and monolingual Spanish speakers, ensuring everyone can access the care they need. Whether through our downtown location, mobile van, or 24/7 supply vending machines and syringe disposal kiosks, we provide:
- Judgment-free access to health services and supplies
- Housing navigation and placement support for people living with HIV
- Benefits navigation for essential needs
- HIV and STI testing, prevention, support and linkage to treatment
- Mental health resources and therapy
- Safe drug use supplies and overdose prevention
- Warmth, kindness and support
