HARM REDUCTION SUPPLY REQUEST FORM

This form can be used by individuals, community groups, and organizations to request harm reduction supplies. Requests are filled based on available inventory.

Once your request is received, a Face to Face staff member will follow up when your order is ready for pickup. Please allow up to one week for processing. If your request is urgent or you need larger quantities than are available through this form, please give us a call and we will do our best to assist you.

Our kits are designed for subcutaneous hormone injection, but we have supplies available for intramuscular injection.
